Alzheimer's & Caregiver
Respite
Enables family caregivers to be temporarily relieved from
their caregiving responsibilities.

Caregiver Support Program . offers information & assistance to caregivers
in gaining access to supportive services to compliment care provided by
the
caregiver and training to assist the caregiver in making decisions and
solving
problems relating to the caregiver role. If you help an older adult with
preparing meals, bathing, dressing, grocery shopping, or doctors visits,
you
are a CAREGIVER.

Community Care Coordination
Is a program designed to coordinate
a variety of in-home services needed by frail elderly persons aged 60
and older who are at risk of entering a nursing home & are not eligible
for PASSPORT.

Energy Assistance . HEAP
applications are available along with
information on telephone discounts and Summer Cool Program.

H.E.M.E.N. . (Helping Elderly
Meet Essential Needs) provides one time or infrequent
services purchased for clients who are determined to be eligible when
all other existing resources fall short.

minor home repair and modifications to provide a safe &
accessible home environment.

Housing Directory for Seniors
. Older adults have a choice of where and how they want to
live. A directory of subsidized senior housing, private pay housing,
assisted living/residential care, independent apartments, congregate
housing, adult care facilities and nursing homes is available.

Information and Referral -
AAA is the starting point for locating local
services and assisting with referrals. Discover choices and resources
available to make good decisions regarding long-term care, to secure and
maintain independence and dignity in a home environment.

Long-Term Care Ombudsman
Program . provides a voice for consumers of long-term care
services by identifying, verifying and resolving concerns
confidentially.
PASSPORT . PASSPORT Home Care Services include case
management, personal care, homemaker, home delivered meals, home medical
equipment, supplies, and more. PASSPORT provides an alternative to
nursing home care. Consumer eligibility is determined by level of need,
financial eligibility and other criteria.
